By completing and submitting this form to the City of San Bruno, the submitting party understands, consents to, and agrees with all of the following terms and conditions:
City of San Bruno makes no warranty of any kind that any of the information contained herein will in all cases be successfully provided to and/or received by any City of San Bruno employee, officer, official, or volunteer who may respond to and/or provide assistance regarding an incident at the business address listed herein.
The submitting party further understands and agrees that the City of San Bruno's possession of any and all of the information contained herein shall not create any duty, contractual relationship, and/or expected course of action upon the City of San Bruno or any of its employees, officers, officials, or volunteers.
The submitting party agrees to indemnify, defend and hold harmless the City of San Bruno and its employees, officers, officials, and volunteers from any and all liability incurred as a result of the City's possession of the information contained herein.
